📋 TRICARE Coverage Policy
Well-Child Visits (Birth – Age 2): Multiple scheduled visits at no cost following the AAP pediatric schedule (newborn, 2 wks, 2, 4, 6, 9, 12, 15, 18, 24 months).
Annual Wellness Visits (Age 3+): Once per year — no cost at your PCM or TRICARE network provider.
School Enrollment Physicals (Ages 6–17): ✅ Covered at no cost when required for school enrollment. The Florida physical form often satisfies both school and sports clearance — confirm with your school's athletic office.
Sports Physicals: ❌ TRICARE does not cover annual sports physicals as a standalone visit.
30-Day Grace Period: You can schedule your next annual exam up to 30 days before the 1-year anniversary of your last one.
Example: Last exam Sept 15 → you can schedule the next one as early as Aug 17.
Urgent Care: Sick or injury visits only — not routine physicals.

📋 Florida School Entry Requirements
📄 Physical Exam
Any K–12 student entering a Florida school for the first time must show a physical exam completed within the past 12 months. Florida recommends Form DH 3040 — ask your clinic for it.
Transferring from another state? A comparable form from that state is fine as long as it's less than 1 year old.

Required by Florida Statute 1003.22 for first-time Florida school entry. Provide proof in one of these ways:
• Electronic records — if your child's Social Security Number is shared with the district
• Florida Certificate of Immunization (Form 680 or 681) — your clinic can issue this

Florida law (Second Chance Act, SB 1070) requires ECG screening for all new 9th-grade student-athletes and first-time FHSAA participants in grades 10–12, effective July 1, 2026. Hurlburt Pediatrics does not perform EKGs — these local providers can help:

Florida Second Chance Act (SB 1070): Effective July 1, 2026, an ECG is required for all new 9th-grade student-athletes and students in grades 10–12 who have never participated in FHSAA-sanctioned sports. An ECG completed on or after July 1, 2024 already satisfies the requirement. Cost must not exceed $50. Returning FHSAA athletes are not required to screen (but are encouraged). Always ask whether a PCM referral is needed before your visit.
